Domestic PSC Benefits Plans

Eligible PSC’s may choose from the health and prescription plan, voluntary dental plan, or other individual benefits.  Please review the outlines below and find further information about these plans, including benefit outlines and summaries, premium costs, payment instructions and forms.

                        Eligibility and Enrollment                   Here, you will find the rules regarding                                                                                 who is eligible to join the PSC Health                                                                                 Plan. Additionally, you can locate all forms and                                                                                 instructions regarding their completion.

                   Health and Prescription Plan                  The current provider features three                                                                                 health plan options from which you                                                                                 may choose.  Prescription coverage                                                                                 is also included in each plan. 

                            Voluntary Dental Plan                   This PPO network coverage features benefits for both 
                                                                                 in and out of network dental service providers.
